I have just a plain old regular diaper bag. It has a few pockets on the outside and reasonable sized inside but it's nothing special.
For dirty diapers I do one of two things. I have a small Bummis PUL bag that I bought especially for this purpose. It holds probably 3, maybe 4 diapers if I really squish them in and has a drawstring top. If it's dirty (I throw it in with my whites to wash it) I just grab a plastic grocery bag to throw the diapers in. I always thought cloth on the go was too much of a pain to bother but really it's not at all that difficult and only slightly more trouble than disposables.
I do admit to using disposable wipes when I go out though. Partly because I have a lot that I have to use up (gotta love those super huge warehouse club sizes!) and partly because it is a little easier if you don't have a handy source of water nearby (like at the park or something). But when I run out I will probably just take my cloth wipes with me along with a small water bottle to wet them.
I've been very lucky in that my son doesn't poop very often when we're out of the house (don't know why, coincidence I guess) but the times I have dealt with it I've usually just stuffed it in the diaper bag to deal with at home. If I put a liner in it, I'll flush that and if I'm at my mom's or mother-in-law's then I'll sometimes dunk if I feel like it.
