By far my favorite is the Airflow... I have tried tons of covers and there is really none that even come to close! They never leak and mine have lasted for years!! My only complaint is that there aren't enough bright prints or colors.
I do have some cute wahm covers that I use for summertime (with no shorts) and I like them fine. They work well for day but honestly I don't know that I'd trust them for night... I have some from Luke's Drawers (www.lukesdrawers.com) that are really cute. And my favorite fleece cover is from Sugar-Peas (www.sugarpeas.com), they're nice and thin, which is rare in a fleece cover (also means that you can't use it at night though).
Just a warning though, these are rather expensive and will not perform as well as the Airflow. I use them because they're cute but my basic and most functional cover is the Airflow.
The Bummis Super Whisper Wraps aren't bad either. They're not quite as good as the ME ones (the elastic around the legs sometimes gets wet and wicks, though not often) and the cute prints only come in velcro. They're only available in two prints at a time though and it takes a long time for them to come out with new ones.
Oh and I can't really comment much for small vs large babies, personally I've never really noticed any cover being any better/worse for either... But then neither of my kids were very skinny or very chubby...
