This has happened with my daughter a couple of times, and each time, even though I thought I had her all tucked into the cover, what I had ALSO done was tuck a TINY bit of her shirt or clothing also into the cover... I figured this out when I saw that she was wet all up her back and the onesie was also kind of folded into the cover. So now I lay the cover down first and then do the diapering in it, then pull the clothes all around it. I just try to be extra careful.
