Do you keep the covers in with the diapers? If so then that's almost surely the problem. Try soaking them in baking soda to solve the problem and don't keep them with dirty diapers. PUL covers will absorb odors when left in prolonged contact with dirty/wet diapers. Just put them somewhere seperate until wash day.
Oh, and the vinegar could be contributing to the problem as well. I know some recomend washing diapers with vinegar but I don't know of any benefit to washing covers with it and I've heard it can be problematic sometimes.
