I'd say your problem is three fold. First you may need to pre-wash your diapers a bit more (4 times is pretty much a minimum so they may not be quite there yet). Second of all you may reconsider the Dreft as others have suggested, the fabric softener in there can actually make diapers water-resistant.

Third and most important of all, yes you definetly need to use a cover at all times. Cloth diapers don't have an exterior waterproof coating like disposables do (unless they're All-In-One diapers that have the cover sewn right on) so the wetness will naturally go right through. No matter how absorbant a diaper is, it's still just fabric so the moisture will go through the layers.... Some occasions may be ok to go without a cover, like if the baby has a rash and you want to air him out.
