It's not anything you've done nor is it a defect. It's normal for terry cloth to pill when it's first washed (just like a brand new towel would). Your prefolds wouldn't have done that because they're not terry cloth. Terry cloth is very soft and fluffy when new because is has lots of surface fibers that will be washed away at first. That's why the instructions advise to only wash about 10 new diapers at a time, so as not to overload the lint trap in your washer. It'll disappear within a few washes and there's no reason why you can wash all your diapers together (even right away).
