I need to buy new AF covers.  My son is about 16lbs.  Right now we are still using size small covers (in the smallest setting at waist, largest at legs) and have our OS diapers folded down in medium size.

Can I get away with buying the size Med-Lg (20-27 lbs) covers?  Or will they be way too big?  I'm thinking they will be too big, since he's still wearing the smalls.  But I wanted a second opinion.

I think the M/L would be a bit big on him but not for long.  He's close to the lower weight limit of 20 lbs.  If you don't mind them going up well past his belly button then you could probably get away with them and have them on the tightest setting on the legs.  

That being said many people skip cover sizes so you could also try going to the medium and then straight to a large from there.  I always found my kids outgrew the covers 3 or 4 pounds before reaching the weight on them so if yours is the opposite then the mediums should hypothetically fit beyond the weight they list.

If you are okay with possibly having extra cover on him, I would think that you'd be able to make the M/L work.  Usually the covers don't have to be particularly snug-fitting, anyway.  The covers can be pretty flexible with sizing, and so much can depend on the build of your child.  Obviously you'd be out the extra shipping, but otherwise you're not gambling much by trying the larger size first, since it's something you could use anyway.  
From another point of view, M/L is the most optional of the cover sizes, as I see it (though I liked having them with my first because of his build, they weren't a necessity and I sold them before DS#2 was out of them because I had plenty of L that worked just fine).  Unless you think that your DS is likely to be on the small side, you'll probably be needing to get L at some point anyway.  If that's the case, there's an argument either for a) just getting some L now and see if they'll work okay for right now and skipping everything else altogether or b) combining shipping and getting both M and L right now so you'll have a trimmer fit for now and the L for when you need them (skipping the M/L).

For what it's worth, I started my son on Lg Airflows (and L. Sandy's) when he was about 17lbs. He was a long and lean baby, no chub at all but he was pretty tall for his age (so the rise might have been less of an issue for him than for others). Now, the covers were way bigger than they needed to be, we used them because we already had them and I couldn't be bothered buying smallers sizes (we didn't discover ME before that except for a couple of Airflows). Very big but they worked just fine and most of the 'bulk' wasn't really 'bulk' as such, just a little extra fabric that's pretty thin. The rest was just air and got squished out under clothing anyway.

Not sure that helps your decision, but it does show that you can usually pretty easily skip sizes with no problems.

I'm wondering how your dc still fits into the smalls at 16 lbs!

I agree that you can skip sizes.  I started my dd in the size mediums at 6 weeks old to save money on the smalls. It added extra bulk but they worked fine.  I switched her to M/L at around 16 lbs and I honestly could have gone straight to large and saved the money.
