I am afraid I am going to have to give up on my mother-ease One sizes.  My 11 month old son has been in them since birth, and I love them.  I use bio-kleen to wash them every other day.  I've never had any problems, until about 3 months ago he started getting a raised red rash, that looks like eczema on his thighs and waist where the binding of the diaper is.  He is chunky, and we already have him on the biggest setting.  I've tried putting in an extra soaker, I already use a fleece liner, I've tried diaper cream and Eucerin.  Nothing works.  I finally took him out of the cloth and put him into a disposable for a week, and the rash has almost gone away.  I tried putting him back in cloth and it came back the same day.  I also stripped the diaper in the meantime with hot water.

Please help.  I love my One-size, and don't want to change, but I don't know what else to do.  Any suggestions would be welcomed.  Thanks!

Motherease has "extender tabs" to give you more room because they are supposed to last from birth to potty training. You have to call and ask for them (I don't believe they are on the website).
I would also try changing more often. I find if my son stays in a wet/poopy diaper too long he will get red marks/rash on his bottom where it rubs because he is so active. Think of it this way, if you had a wet "towel" rubbing you body constantly, wouldn't you get a sore?
I love my one size and sandies too, so I understand not wanting to have to give up on the one size.

Have you tried putting the diaper either looser or tighter? If it's too tight then that can cause irritation and similarly if it's too loose (being too loose may cause it to shift and move more).

Have you tried another diaper, a Sandy's perhaps? Maybe it's just the binding on the One-Size that isn't working all that great and another style of diaper would work better?

Have you showed the rash to a doctor to see what he/she thinks? Sometimes persistant rashes can be something like yeast and once treated with anti-fungals disappear completly. My dd had a nasty rash that would come/go for months, I would treat the heck out it with all kinds of methods, including disposables for days, even weeks. The rash would go away, I'd put her back in cloth and the rash would come back. Finally had a doc look at it, she gave us a prescription for an anti-fungal & cortisone cream and it disappeared completly. Some also just use over-the-counter creams for female yeast infections, they're apparently pretty much the same thing.

Agree with having a doctor look at it, but I've become hesitant to use prescription creams (past experience with recurring problems).
Perhaps you could try using colloidal silver; either spray some on or use the gel. I've used this with success. Has anyone else tried this? I'd like to know what others have learned about colloidal silver.
