Update:  Ok-I am back from the doctor and I have to give Holly 1/2 capsule once a day of Cultrelle Probiotic.  I understand this is to help her get back the good bacteria into her intestines and to slow down her pooping. She poops about 3 or more times in a 30 minute period which is way to much.  Also, a cream that I need to mix up is called Wedel Cream, (named after a pediatrican here) the ingredients are:
1/2 tube of A&D
1/2 Tube of Desitin
1 Tbsp of Maalox
1 Tbsp of Triple Antibiotic (Neosporin)

I am going to try this to see if it works.  Thought I would share my news.

This is a update on the Posting "Diaper Rash". I have been reading all the awesome posts and useful info on my topic of Diaper Rash. To clear a few things up I have tried using disposables and all it does is make the itial rash area redder.  Once I put her back into cloth, I use 100% Indian cotton prefolds on her, the redness fades within one diaper change. I also use warm water and cloth wipes too.

So....what I have tried now is the recommendation of my lactation consultant who I see twice a week for weigh ins.  She said to try an anti-fungal cream since the spots have lasted for 4 weeks. I guess I will try anything now because red spots that last for 4 weeks and it just doesnt seem right.   I am posting two pictures on Photobucket so everyone can actually see what I am seeing and maybe get a better idea of what it is.  I am sorry if a picture of my baby girls private parts offends anyone but I doing this in hopes that someone might figure out what this is and how to treat it. Also there is a password you have to use to view this.  I did this for added security...I dont want just anyone to see her bottom. it is "sorebum"

I have to agree that this isn't normal at all, so basically ignore my previous suggestion that perhaps it was 'normal' to a certain extent...

An anti-fungal does seem like a good place to start, regardless of what the initial rash was it could well have turned into a yeast rash since then. If yeast becomes the #1 suspect and it turns out to be a tough infection you may need to consider treating the diapers as well. Most germs and such are washed away with normal hot water washes but yeast could be possibly much tougher as it can withstand normal hot water. In theory you need anti-fungal agents (like tea tree oil or bleach) or very high heat (boiling water or steam) to truly get rid of yeast.

I would recomend maybe boiling or ironing your diapers since you mention you're using prefolds. With fitted diapers this would be a bit of a risk since high heat isn't good for snaps or elastic but with prefolds it shouldn't be a problem. You could use tea tree oil in the wash (20-30 drops I've heard as a yeast treatment) and then rinse really well to make sure it's all rinsed out of the diapers. The only real guarantees though are probably bleach or very high heat. Tea tree oil can be an effective anti-fungal but I don't know that it's 100% effective (I don't think it's been studied basically).

I don't know that these steps are necessary as some people (myself included when my daughter had a yeast rash and I didn't know any of this) effectively deal with yeast and never do any of this but it's worthwhile knowing because it is possible to pass such an infection back/forth between skin and diapers.

ya i noticed the password didnt work too. i must have done something wrong.  oh well.  I put a  call into the dr. to see if there is anything else I can do.  Maybe they will see her again and try something else.  A rash like that for 4 weeks seems unreal and not fair to her at all.  Her dr. first suggested hydrocortizone 1% cream until the rash went away, but all that did was make her scream and caused the area to get very very red and the sores actually got bigger.  Thanks Karen for the info.

I posted before and recommended tea tree  because it sounded like yeast and now that I've seen the picture I am sure. It looks just like my babies yeast rash. I would go coverless, so the yeast can dry out. Keep the bottom as dry as possible, because the yeast will grow faster in a warm wet place. As I recommended before one drop of tea tree on a wet cloth wipe every time you wash her. Tea tree will kill the yeast. Start with one, you can probably put more drops then that but just be careful not to make her skin burn. I'm remembering what my midwife told me, she said with yeast you don't want to put all sorts of creams, becauce they don't let the skin air, which can breed more yeast. She also said, try not to wipe the skin but instead dab the skin. Because lots of wiping can cause the skin to be even more sore and raw.

Ok. I do have a bottle of tea tree oil that I use for her baby wipes solution. I have only been putting 2 drops of oil per 2 cups of water.  But you say to use one drop of oil on a wipe for each diaper change.  I have a 2oz spray bottle could I just mix together...say 20 drops of tea tree oil and 1.5 oz of distilled water and shake that prior to spraying directly onto her bottom.  Do you think that would work?

Yes, that would work. I would make sure that her skin is completely dry before putting she diaper back on though.

My baby boy had exactly that same rash for 5+ weeks as a newborn when he was pooping nonstop! I had to stop using Pampers Sensitive wipes and make cloth wipes. In the end what worked for us was a mixture of distilled water, a squirt of mild baby wash and a couple droppersful of grapefruit seed extract shaken up in a peri bottle and sprayed on the wipes. Hope your baby's bum heals soon mama!
